New Program Offerings for 2015
Advocacy Program: When a patient is facing a new medical diagnosis, prognosis, and/or available treatment options, it’s common to feel confused. Making decisions can be very difficult as you navigate the healthcare system, especially when you need to choose between several options. During your one-hour appointment in the Employee Health Center the Riverview Health provider will review your medical information and provide a second opinion or guidance of next steps. It is highly encouraged to bring another family member or loved one to your appointment and any necessary medical records, x-rays or prescriptions. The program does not require a provider referral and can be scheduled below.
Medication Therapy Management (MTM) is a service provided by an experienced Riverview Health pharmacist who sits down with you in a confidential surrounding to provide an in-depth review of all of your medications. The goal is to ensure that your current drug therapy is both safe and effective. During this time with your pharmacist, any questions that you have about your medications can be answered. The pharmacist will work directly with your provider to resolve any identified problems with your medications. During your next visit to the Employee Health Center ask the provider if this program is for you. The program is offered only by provider referral only.
Respiratory Therapy Education: Asthma and COPD (emphysema) patients may need to see a respiratory therapist to learn about proper medication use, treatment options or education about the disease process. This interactive education program assists patients in preventing urgent care or emergency room visits. During your next visit to the Employee Health Center ask the provider if this program is for you. The program is offered only by provider referral only.
Wellness Coaching: Professional wellness coaches help people improve their overall health and well-being by:
If you are struggling with losing weight, being more active, managing stress, quitting tobacco or another health and wellness area and are committed to making long-term changes, wellness coaching maybe for you. The program is NOW offered by both self and provider referral. An initial wellness coaching session, should be scheduled first. Scheduling an Office Appointment
Regular appointments are available in 20-minute increments, child or adult physicals or PAP tests require TWO consecutive appointment times (40 minutes total). If the patient is in need of a high school sports physical, please schedule with an M.D. Advocacy Program consults are 60-minute appointments and are only scheduled with an M.D. automatically. Wellness Coaching initial and follow-up appointments can be self-scheduled using the scheduling options below, but an initial appointment MUST be scheduled first, before a follow-up appointment.
If you are scheduling an appointment for a vaccine(s), it is required to bring all childhood or appropriate adult vaccine records with you to your appointment.
